A program for Wisconsin’s frail elders and adults with disabilities.
IRIS (Include, Respect, I Self-Direct) is a self-directed program for Wisconsin’s frail elders and adults with disabilities. You have to be eligible for Medicaid to be in the IRIS program.
IRIS is built on the principles of self-determination and self-direction. That means that you will have the freedom to direct your cares.
Each participant has a customized Service and Support Plan and a budget. This Plan outlines the services, goods, and supports on which they can spend the budget. What can be included in the Plan is broadly defined as anything that helps the individual to maintain their independence provided the cost does not exceed the cost for the same item or service provided under other Medicaid programs.
If you are an IRIS patient, please contact your IRIS representative for approval. Once you have approval please call (414) 305-2633 to book your appointment.
